- Send a one-time acknowledgement email when you submit the online appointment form

Appointment form submissions
When you submit the appointment form on this site, your information is stored in a secure database hosted by Cloudflare. We use Resend to send confirmation emails. Your Turnstile verification is processed by Cloudflare to prevent automated submissions. None of these services receive more information than is necessary to complete the transaction.
Cookies
This site does not use tracking cookies or advertising cookies. Cloudflare may set functional cookies for security and performance purposes as part of its infrastructure services.
Data retention
Appointment request records are retained for a reasonable period to support your service history. You may request deletion of your information by calling us at (503) 465-2926.
Children
This site is not directed at children under 13. We do not knowingly collect personal information from children under 13.
